Why documentation matters in Ayurvedic practice
Ayurvedic documentation serves multiple purposes simultaneously: clinical continuity (ensuring any practitioner can understand and continue a patient treatment), regulatory compliance (DHA and MOH requirements), and business integrity (linking treatment records to billing).
Ayurvedic clinics that document digitally from the start build a clinical history that supports better patient care as the relationship develops over months and years.
The initial consultation: constitution and assessment
The Ayurvedic initial consultation typically includes a constitutional assessment — determining the patient Prakriti (natural constitution) and Vikriti (current imbalance). This assessment informs the treatment plan.
Digital documentation of the initial assessment should capture constitutional findings, presenting complaints in both Ayurvedic and conventional clinical terms (useful for regulatory reporting), and the proposed treatment approach.
Session-by-session treatment records
Each treatment session — whether Abhyangam, Shirodhara, Basti, or any other Panchakarma procedure — should be documented with the therapist performing the treatment, the duration, the materials used, the patient response, and any observations for the supervising physician.
This session-level documentation feeds into the course summary and is the basis for the clinical record submitted for any regulatory purposes.

Treatment course summaries
At the end of a treatment course, a summary document should be generated capturing the total sessions completed, the primary treatment protocols applied, the overall clinical response, and the recommended next phase.
For referring physicians — particularly for patients referred from conventional medical practices — this summary should be formatted to be understood by a clinician not familiar with Ayurvedic terminology.
